Transition Services Coordinator (Austin, Texas): Provide coordination of transition services to young adults and others with disabilities.  Services include independent living skills instruction, peer support, information, and referral to persons with disabilities in Travis and surrounding counties. Good communication, problem-solving, and remote computer skills. Related degree (experience may substitute), Salaried Full time ($22.44 per hour 36 hours per week). Independent Living Health Support Specialist

The Independent Living Health Support Specialist reports to the Deputy Director and works as a team member, with moderate supervision. Responsible for the management of public health projects geared toward persons with disabilities.

Develops strategies that boost social support, such as supportive social networks, partnerships, and similar actions that can help respond to COVID-19 or prepare the disabled community for other public health challenges. Creates and disseminates public health messages, eliciting feedback from individuals and groups.  Responds to requests for information and referrals from consumers, their families, and the community.

Maintains detailed records using the CILSuite data entry system.

Salaried Full time ($26.70 per hour 36 hours per week).
